Pengelolaan Diabetes Mellitus (Tipe 2) salah satunya dengan diet seimbang. Kendala penanganan diet Diabetes Mellitus adalah kejenuhan pasien mengikuti terapi diet dan kurangnya dukungan keluarga. Diabetes Mellitus atau kencing manis adalah penyakit gangguan metabolisme gula darah yang disebabkan abnormalitas hormon insulin sehingga terjadi peningkatan kadar gula darah. <\/p>\n\n\n\n<p>Semua zat gizi sangat penting dalam diet diabetes. Makanan sumber karbohidrat harus dibagi merata di sepanjang hari untuk mengimbangi insulin yang mampu diproduksi oleh tubuh. Gejala penyakit diabetes mellitus diantaranya meningkatnya rasa haus, dehidrasi, gangguan elektrolit dan penurunan berat badan. Untuk mengimbangi tidak tersedianya glukosa sebagai sumber energi, tubuh akan meningkatkan laju pemecahan glikogen serta lemak untuk melepaskan sumber-sumber energi dan memproduksi glukosa dari hasil pemecahan protein tubuh.<br><br><br>Tujuan terapi diet diantaranya :<\/p>\n\n\n\n<ol class=\"wp-block-list\"><li>Mencegah terjadinya hiperglikemia namun masih memberikan energi cukup<\/li><li>Memulihkan dan mempertahankan kadar glukosa darah dalam kisaran nilai yang normal (gula darah puasa &lt; 126 mg\/dl)<\/li><li>Memulihkan dan mempertahankan berat badan yang normal<\/li><\/ol>\n\n\n\n<p>Makanan yang dianjurkan untuk penderita diabetes diantaranya ayam tanpa kulit, ikan, putih telur, daging tidak berlemak. Sumber protein nabati yang dianjurkan diantaranya tempe, tahu, kacang hijau, kacang merah, kacang tanah, kacang kedelai, sayuran yang diperbolehkan diantaranya kangkung, daun kacang, oyong, ketimun, tomat, labu air, kembang kol, lobak, sawi, selada, seledri, terong. Buah-buahan seperti jeruk, apel, pepaya, jambu air, salak dan belimbing diperbolehkan untuk dikonsumsi.<br><br>Semua jenis karbohidrat seperti nasi, bubur, roti, mie, kentang, singkong, ubi, sagu, gandum, pasta, jagung, talas, havermout, sereal dan kentang diperbolehkan namun dibatasi sesuai kebutuhan. <strong>Enam langkah makan sehat bagi penderita diabetes<\/strong> diantaranya :<\/p>\n\n\n\n<ol class=\"wp-block-list\"><li><strong>Makan tiga kali sehari<\/strong> dan jangan lewatkan waktu makan<\/li><li>Lengkapi setiap porsi makan dengan makanan k<strong>arbohidrat yang lebih kompleks<\/strong> meliputi roti gandum, oat, dan kentang.<\/li><li>Makan <strong>lebih banyak buah dan sayuran<\/strong>. Makan 3 \u2013 5 porsi sayur sehari secara perlahan namun teratur.<\/li><li>K<strong>urangi gula dan makanan manis<\/strong>. Diet bebas gula tidak perlu benar- benar dipatuhi dengan ketat, gula dapat dipakai sebagai salah satu bahan didalam makanan, misalnya didalam sereal sarapan gandum utuh. Konsumsi maksimum gula sebesar 5% dari total kebutuhan energi sehari. Minuman manis dapat diganti dengan minuman bebas gula.<\/li><li><strong>Kurangi garam <\/strong>dengan membatasi jumlah asupan makanan olahan serta garam tambahan. Rempah dan bumbu dapat digunakan sebagai alternatif.<\/li><\/ol>\n\n\n\n<p>Dalam melaksanakan diet, penderita DM tipe 2 harus mengikuti anjuran <strong>3J, yaitu jumlah makanan, jenis makanan dan jadwal makanan<\/strong>.<br>Jenis dan jumlah makanan yang banyak mengandung gula serta jadwal makan yang tidak teratur dapat meningkatkan kadar gula darah. Jumlah kalori basal yang besarnya 25-30 kalori\/kgBB ideal, bergantung pada jenis kelamin, umur, aktivitas, dan status gizi. Kebutuhan kalori pada wanita lebih kecil daripada pria. Kebutuhan kalori wanita sebesar 25kal\/kgBB dan untuk pria sebesar 30kal\/kg BB. <br><br>Penurunan kebutuhan energi untuk usia &gt; 40 tahun menurut PERKENI tahun 2011 dengan ketentuan usia 40-59 tahun, kebutuhan energinya dikurangi 5% dan usia 60-69 tahun kebutuhan energinya  dikurangi 10%, usia &gt;70 kebutuhan energinya dikurangi 20% dari<br>kebutuhan energi. <br><br>Perlu adanya p<strong>embatasan makanan dengan indeks glikemik tinggi<\/strong> karena indeks glikemik yang tinggi pada makanan dapat mempengaruhi kadar glukosa darah 2 jam setelah makan. Contoh makanan yang memiliki indeks glikemik tinggi yaitu roti putih, minuman bersoda, dan nasi putih. Makanan dengan indeks glikemik rendah memberikan manfaat tidak hanya untuk glikemik postprandial tetapi juga untuk lipid. <br><br>Contoh beberapa makanan dengan indeks glikemik rendah diantaranya beras merah, gandum, sayuran, dan kacang-kacangan merupakan sumber karbohidrat yang kaya akan serat. Sumber protein rendah lemak seperti ikan, ayam tanpa kulit, susu skim, tempe dan tahu. Penderita diabetes mellitus makan sesuai jadwal, menurut waspadji tahun 2017 pembagian waktu makan yaitu 3 kali makan utama, 3 kali makan selingan dengan interval waktu 3 jam yaitu makan pagi pukul 07.00, selingan pagi pukul 10.00, makan siang 13.00, selingan siang pukul 16.00, makan sore pukul 19.00 dan selingan sore pukul 21.00.
