Mari kita simak pada artikel berikut.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Definisi<\/h2>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">Diet merupakan determinan utama dari beban asam yang harus diekskresikan oleh ginjal untuk menjaga keseimbangan asam basa. <\/span><\/strong>Asupan memiliki potensi untuk berkontribusi terhadap terjadinya asidosis metabolik ringan dan mempengaruhi status asam basa tubuh melalui penyediaan prekusor asam dari makanan.<span id=\"5a18cc5b-91a1-48fd-84f7-28bfdd3ecf1a\" data-items=\"[&quot;2128164275&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b1\u200b<\/sup><\/span><\/p>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\"><em>Dietary acid load<\/em> didasari oleh pola makan seseorang yang terdiri dari bahan makanan yang mengandung prekusor asam, dan memproduksi asam sebagai hasil dari metabolismenya.<\/span><\/strong> Asupan yang mengandung prekusor basa dan memproduksi basa sebagai hasil metabolismenya berfungsi untuk mengimbangi beban asam harian. Namun, efek akhir dari asupan <em>dietary acid load<\/em> ini adalah bertambahnya beban asam dalam tubuh yang harus di buffer dan dikeluarkan untuk menjaga keseimbangan asam basa. Asam endogen diproduksi ketika sulfur organik yang ditemukan pada asam amino metionin dan sistein dioksidasi menjadi sulfat anorganik. Asam tambahan diproduksi ketika makanan netral teroksidasi menjadi anion organik yang diekskresi di urin, termasuk sitrat, <a href=\"https:\/\/ahligizi.id\/blog\/2021\/07\/19\/bahan-makanan-yang-terkait-kadar-asam-urat\/\">asam urat<\/a>, dan oksalat. Selain asam tambahan yang diproduksi secara endogen dari makanan, asam dan basa eksogen juga langsung diserap di saluran pencernaan.<span id=\"7c1686f2-6396-4e95-a354-d0e902557c39\" data-items=\"[&quot;4220530966&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Asupan makanan dapat mempengaruhi produksi asam endogen, bukan karena pH dari makanan namun karena asam non volatil dan prekusor basa (senyawa yang menghasilkan asam atau basa setelah makanan diserap dan di metabolisme). Apabila asupan lebih banyak mengandung prekusor asam daripada prekusor basa dapat menyebabkan metabolisme asidosis ringan. Kebiasaan diet seperti ini menyebabkan beban asam pada tubuh yang berasal dari makanan kurang lebih 1 mEq\/kg\/hari40. Sebuah penelitian juga telah melaporkan bahwa sumbangan prekusor asam dari makanan sekitar 50-75 mEq\/hari pada beberapa studi kohort.<span id=\"7e132fb1-a831-4496-9c8e-05846ac6103d\" data-items=\"[&quot;985437951&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b4\u200b<\/sup><\/span><\/pre>\n\n\n\n<h2 class=\"wp-block-heading\">Kaitan <em>Dietary Acid Load<\/em> dan pH Urin<\/h2>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">Diet yang mengandung beban asam (<em>Dietary Acid Load<\/em>) yang tinggi, mampu menyebabkan terjadinya asidosis metabolik ringan dengan cara mempengaruhi status asam basa tubuh melalui penyediaan prekusor asam dan basa dari makanan<\/span><\/strong>. Asidosis metabolik memperbanyak sekresi glukokortikoid dan meningkatkan konsentrasi plasma kortisol, yang mungkin menimbulkan resistensi insulin. Selain itu tingginya <em>dietary acid load<\/em> juga mampu menurunkan nilai pH darah, penelitian terdahulu menunjukan penurunan pH darah pada subjek yang sehat dapat mengurangi sensitivitas insulin. pH urin dapat dijadikan tolak ukur untuk menggambarkan kondisi kesimbangan asam basa tubuh.<span id=\"f18aaf5e-309d-482a-b018-c444876f12ce\" data-items=\"[&quot;163432171&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b5\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Keseimbangan asam basa dalam tubuh dipertahankan oleh 3 mekanisme, yaitu sistem penyangga darah dan jaringan, ekskresi CO<sub>2<\/sub> oleh paru-paru dan ekskresi H<sup>+<\/sup> dan regenerasi HCO<sub>3<\/sub> oleh ginjal. Beberapa efek yang diketahui apabila tubuh mengalami asidosis metabolik diantaranya adalah <em>nitrogen wasting<\/em>, penurunan level IGF-1, pembentukan batu ginjal, hipotiroidisme dan hiperglukokortikoidisme ringan.<span id=\"89ed2758-324b-48ff-886a-4ad1ff9687a5\" data-items=\"[&quot;777306679&quot;]\" contenteditable=\"false\" class=\"abt-citation\" data-has-children=\"true\"><sup>\u200b6\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Asupan berkontribusi terhadap keseimbangan asam basa melalui metabolisme sulfur yang mengandung asam amino sistein dan metionin di hati. Metabolisme ini menghasilkan ion H<sup>+<\/sup> dan menurunkan pH. <strong><span class=\"has-inline-color has-vivid-red-color\">Metabolisme garam alkalin pada buah dan sayur yang mengandung karbonat dan asam organik dan suplai mineral magnesium dan kalium dalam jumlah besar, menyeimbangkan efek dari ion H<sup>+<\/sup>  dalam tubuh dan meningkatkan pH<\/span><\/strong>. <strong><span class=\"has-inline-color has-vivid-red-color\"><em>Dietary acid-base load<\/em> merupakan keseimbangan antara asupan kaya protein seperti daging, sereal, susu dan produk olahannya yang menyumbang asam dengan sayur dan buah yang menyumbang basa dalam tubuh<\/span><\/strong>. Ginjal merupakan rute utama untuk mengekskresi ion H<sup>+<\/sup>  dari makanan, dan <em>acidbase load <\/em>dari asupan berkaitan dengan urin pH.<span id=\"145f5dc8-6895-4dc2-9fde-03b79254ad9a\" data-items=\"[&quot;777306679&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b6\u200b<\/sup><\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Hubungan <em>Dietary Acid Load<\/em> dan <a href=\"https:\/\/ahligizi.id\/blog\/2021\/07\/07\/hubungan-dietary-inflammatory-index-dengan-obesitas\/\">Obesitas<\/a><\/h2>\n\n\n\n<p>Keseimbangan asam basa yang terganggu juga berakibat pada peningkatan berat badan. Ion hidrogen dapat menghambat produksi energi di mitokondria melalui penghambatan sikus asam trikarboksil dan mengakumulasi asam laktat pada subjek yang memiliki pola makan <em>Western Diet<\/em>. <strong><span class=\"has-inline-color has-vivid-red-color\">Kalori dan <em>acid load<\/em> yang terakumulasi dari pola makan <em>Western Diet<\/em>, bisa menghasilkan laktasidosis apabila tidak dikompensasi secara efektif dan efisien melalui rute pernafasan.<\/span><\/strong> Selain itu, produksi asam keton endogen juga akan menurun, sehingga lemak akan terakumulasi di jaringan adiposa dan terjadi penurunan oksidasi asam lemak. <strong><span class=\"has-inline-color has-vivid-red-color\">Keadaan asidosis menyebabkan adanya gangguan pada pemecahan asam lemak dan kecenderungan untuk mensintesis lemak yang lebih besar.
