Hal ini tidak disebabkan oleh penyakit neurologis lain, kejadian yang jarang terjadi dan merupakan kondisi paling parah dari semua gangguan hipertensi selama kehamilan dengan risiko kematian ibu dan janin yang tinggi.<span id=\"f184dc99-7a14-4016-b028-1cd0dddc228c\" data-items=\"[&quot;3191923363&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2\u200b<\/sup><\/span><\/p>\n\n\n\n<p><a href=\"https:\/\/ahligizi.id\/blog\/2021\/01\/29\/alami-heartburn-saat-hamil-berikut-cara-mengatasinya\/\"><strong>Lebih Lanjut : Cara Cegah Heartburn Saat Hamil<\/strong><\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Dampak Pre Eklampsia dan Eklampsia<\/h2>\n\n\n\n<p>Ibu dengan preeklampsia ringan mungkin tidak merasakan dampak yang begitu besar. Namun, ibu yang mengalami <strong><span class=\"has-inline-color has-vivid-red-color\">preeklampsia berat dapat mengalami gangguan pada hati, ginjal, otak, dan gangguan pada sistem pembekuan darah, biasanya bersifat dinamik, progresif, bahkan <em>irreversible<\/em>. <\/span><\/strong>Morbiditas berat yang berasosiasi dengan preeklampsia, di antaranya gagal ginjal, stroke, gagal jantung, <em>adult respiratory distress syndrome<\/em>, koagulopati, dan gagal hati. <\/p>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">Ibu yang mengalami preeklampsia akan meningkatkan risiko 3-25 kali untuk terjadinya komplikasi obstetri yang berat.<\/span><\/strong> Dampak dari ibu yang mengalami preeklampsia pada janin dapat menyebabkan:<span id=\"844cb0db-6f4d-4e02-a91f-c67f750b76f3\" data-items=\"[&quot;207898505&quot;,&quot;2657070437&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b3,4\u200b<\/sup><\/span><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Kelahiran prematur<\/li><li>Pertumbuhan janin terhambat<\/li><li>Oligohidramnion<\/li><li>Abrupsi plasenta (pelepasan prematur pada plasenta, plasenta lepas saat janin masih di dalam kandungan)<\/li><li>Kematian janin<\/li><\/ul>\n\n\n\n<p>Kejang dapat terjadi pada eklampsia. Selain itu juga terjadi gejala sindrom HELLP\/<em>hemolysis,elevated liver enzyme,low platelet count <\/em>(hemolisis, peningkatan enzim hati, trombosit rendah). Kasus yang parah, mungkin juga terdapat ruptur hati, edema paru, gagal ginjal akut, abrupsi plasenta, peningkatan kreatinin, hambatan pertumbuhan intrauterin, perdarahan otak, kebutaan kortikal, dan ablasi retinal. <strong><span class=\"has-inline-color has-vivid-red-color\">Angka kematian ibu jika mengalami eklampsia meningkat sekitar 8-36%, mempersulit sekitar 1 dari 2000 kehamilan.<\/span><\/strong><span id=\"a778d377-776c-496d-92a7-9a31a6012824\" data-items=\"[&quot;207898505&quot;,&quot;593313322&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b3,5\u200b<\/sup><\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Tanda dan Gejala Pre Eklampsia dan Eklampsia<\/h2>\n\n\n\n<p>Gejala ringan pada Pre Eklampsia :<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Tekanan darah sekitar 140\/90 mmHg atau kenaikan tekanan darah 30 mmHg untuk sistolik atau 15 mmHg untuk diastolik dengan intervenal pengukuran selama 6 jam<\/li><li>Protein urin melebihi 0,3 g\/liter dalam 24 jam atau tes urin dipstick \u22651<\/li><li>Bengkak kaki, tangan, atau lainnya (edema)<\/li><li>Kenaikan berat badan lebih dari 1 kg\/minggu.<\/li><\/ul>\n\n\n\n<p><em>Signs &amp; symptomps severe pre eclampsia<\/em> :<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Tekanan darah 160\/110 mmHg atau lebih<\/li><li>Pengeluaran protein dalam urine lebih dari 5g\/24 jam<\/li><li>Penurunan produksi urine kurang dari 400cc\/24 jam<\/li><li>Edema paru dan sianosis (kebiruan)<\/li><li>Sesak napas, terdapat gejala subjektif (sakit kepala, gangguan penglihatan seperti penglihatan kabur, nyeri pada perut bagian atas).<\/li><\/ul>\n\n\n\n<p>Eklampsia merupakan keadaan dimana ditemukan serangan kejang tiba-tiba yang dapat disusul dengan koma pada wanita hamil, persalinan, atau masa nifas yang menunjukan gejala preeklampsia sebelumnya. Umumnya serangan kejang didahului dengan memburuknya preeklampsia dan terjadinya gejala-gejala nyeri kepala di daerah frontal, gangguan penglihatan (pandangan kabur), mual keras, nyeri di daerah epigastrium, dan hiperrefleksia. Menyaring semua kehamilan primigravida, terutama ibu hamil dengan usia \u2264 20 tahun dan semua ibu hamil dengan risiko tinggi terhadap preeklampsia dan eklampsia.<\/li><li>Waspada terhadap kemungkinan terjadinya preeklamsia dan eklamsia bila terdapat faktor prediposisi. Melakukan pemeriksaan antenatal care secara teratur yaitu minimal 4 kali kunjungan yaitu masing-masing 1 kali pada trimester I dan II , serta 2 kali pada trimester III.<\/li><li>Istirahat cukup,  Berdasarkan penelitian yang didapat dari Cochrane,<strong><span class=\"has-inline-color has-vivid-red-color\"> istirahat di rumah 4 jam\/hari dapat menurunkan risiko preeklampsia dibandingkan tanpa pembatasan aktivitas.<\/span><\/strong><\/li><li>Menjaga kenaikan berat badan. <strong><span class=\"has-inline-color has-vivid-red-color\">Sebelum hamil, apalagi kalau pernah preeklamsia sebelumnya, perlu melakukan penurunan berat badan.<\/span><\/strong><\/li><li>Diet rendah garam, lemak serta karbohidrat dan tinggi protein. <strong><span class=\"has-inline-color has-vivid-red-color\">Peningkatan asupan serat dan kalsium telah dikaitkan secara positif dengan penurunan risiko.<\/span><\/strong> Beberapa penelitian menyebutkan wanita yang berisiko harus didorong untuk mengkonsumsi produk susu, buah-buahan dan sayuran yang memadai sebelum dan selama kehamilan.<\/li><li>Pada hipertensi kronis, mengontrol tekanan darah sebelum konsepsi dan pengendalian diabetes melitus sebelum konsepsi dan selama kehamilan.<\/li><li>Suplementasi kalsium selama kehamilan untuk mencegah preeklampsia dan komplikasinya. Jika wilayah dimana konsumsi kalsium rendah, <strong><span class=\"has-inline-color has-vivid-red-color\">suplementasi kalsium selama kehamilan (dosis 1,5-2 gram kalsium elemental\/hari) direkomendasikan untuk mencegah preeklampsia pada semua perempuan terutama pada kelompok dengan resiko tinggi preeklampsia.<\/span><\/strong><\/li><\/ol>\n\n\n\n<p><strong><a href=\"https:\/\/ahligizi.id\/blog\/2019\/11\/22\/diet-pada-ibu-hamil\/\">Baca : Diet pada Ibu Hamil<\/a><\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Manajemen Gizi pada Preeklampsia<\/h2>\n\n\n\n<ul class=\"wp-block-list\"><li>Pertahankan diet sesuai usia dan tahap kehamilan (umumnya 300 kkal lebih banyak dari diet pra-kehamilan). Gunakan ekstra buah dan sayuran dan kurangi sukrosa. <\/li><li>Studi juga menunjukkan efektivitas magnesium pada eklampsia dan preeklamsia. Sumber magnesium termasuk sayuran berdaun hijau, kacang-kacangan, kacang-kacangan, dan biji-bijian. <\/li><li>Asupan <a href=\"https:\/\/ahligizi.id\/blog\/2020\/09\/30\/perhitungan-konsumsi-garam-individu\/\">natrium<\/a> mungkin perlu dikontrol hingga 2 g\/hari jika edema parah. Diuretik umumnya tidak digunakan. <\/li><li>Asam lemak <a href=\"https:\/\/ahligizi.id\/blog\/2020\/11\/11\/fungsi-omega-3-dan-6\/\">omega-3<\/a> yang cukup dari tuna,<strong>&nbsp;<\/strong>tenggiri, tongkol, gabus, kembung, teri, mujair, kedelai, dll.<\/li><li>Diskusikan sumber kalsium yang cukup dari makanan, terutama jika produk susu tidak ditoleransi atau disukai.<\/li><li>Melewatkan makan harus dihindari dengan cara apa pun. <\/li><li>Sumber potasium dan magnesium yang baik termasuk kacang-kacangan, buah-buahan dan sayuran. Diet DASH adalah diet yang sangat baik untuk dilanjutkan, bahkan setelah kehamilan. <\/li><li>Istirahat sangat penting selama waktu ini. Biofeedback, yoga, meditasi, dan bentuk pengurangan stres lainnya seringkali bermanfaat. <\/li><li>Karena homosistein plasma sering meningkat, asupan asam folat, vitamin B12 dan B6 yang cukup dianjurkan. Suplementasi dengan vitamin prenatal. Sertakan asam folat, kalsium, vitamin B kompleks lainnya, protein, selenium, dan kalium dari makanan. Peran vitamin C dan E tidak menguntungkan bagi wanita yang berisiko mengalami preeklamsia.
