Resisten insulin adalah turunnya kemampuan insulin untuk merangsang pengambilan glukosa oleh jaringan perifer dan untuk menghambat produksi glukosa oleh hati. Sel \u03b2 tidak mampu mengimbangi resistensi insulin ini sepenuhnya, artinya terjadi defensiensi relatif insulin. Ketidakmampuan ini terlihat dari berkurangnya sekresi insulin pada rangsangan glukosa, maupun pada rangsangan glukosa bersama bahan perangsang sekresi insulin lain.<span id=\"7c6a1cc5-0143-4696-a680-bac4ff8712d6\" data-items=\"[&quot;1418186963&quot;]\" contenteditable=\"false\" class=\"abt-citation\" data-has-children=\"true\"><sup>\u200b1\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Terdapat <strong><span class=\"has-inline-color has-vivid-red-color\">empat pilar penanganan diabetes mellitus tipe 2 (DMT2) di Indonesia yaitu dengan edukasi, pengaturan makan, aktivitas fisik, dan intervensi farmakologis. <\/span><\/strong>Pengaturan makan merupakan komponen utama dalam keberhasilan penatalaksanaan DMT2. Prinsip pengaturan makan pada pasien DMT2 yaitu makan makanan yang seimbang dan sesuai dengan kebutuhan kalori dan zat gizi masing-masing individu dengan diberikan <strong><span class=\"has-inline-color has-vivid-red-color\">penekanan mengenai pentingnya jenis dan jumlah kalori, serta keteraturan jadwal makan. Kelebihan produksi ROS (yang merupakan oksidan) mampu meningkatkan kondisi stres oksidatif dan hal tersebut dapat memicu penurunan kapasitas antioksidan dalam darah.<\/span><\/strong> Ketika kapasitas antioksidan tidak mampu mengimbangi oksidan yang terbentuk, salah satu akibatnya adalah dapat meningkatkan peroksidasi lemak. Sepasang askorbat radikal yang terbentuk dari reaksi ini akan dengan cepat bereaksi menghasilkan satu molekul askorbat dan satu molekul dehidroaskorbat. Tugas utamanya adalah melindungi kerusakan membran akibat dari serangan oksidan dengan cara mendonasikan atom hidrogennya untuk menteralkan asam lemak peroksil radikal sehingga akan mengakhiri proses peroksidasi lemak. \u03b1-tokoferol yang<br>telah kehilangan atom hidrogen akan dikembalikan menjadi bentuk \u03b1-tokoferol dengan bantuan vitamin C. <strong><span class=\"has-inline-color has-vivid-red-color\">Sumber makanan yang mengandung vitamin E diantaranya kacang-kacangan, bayam, brokoli, alpukat, selai kacang, telur, daging sapi, dan daging ayam.<\/span><\/strong><span id=\"c2179bdd-20e2-48f1-a966-6b07f6f85285\" data-items=\"[&quot;785000060&quot;,&quot;1107249092&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2,4\u200b<\/sup><\/span> <\/p>\n\n\n\n<p>Sebuah penelitian menunjukkan adanya efek protektif dari asupan vitamin E dalam melawan kejadian DM tipe 2, namun penelitian lebih lanjut perlu dilakukan untuk mengetahui mekanisme asupan vitamin E dan metabolismenya dalam mempengaruhi toleransi glukosa dan sensitivitas insulin maupun sekresi insulin.<span id=\"93d47280-fa98-4a1c-ada2-e58596340695\" data-items=\"[&quot;2285860896&quot;]\" contenteditable=\"false\" class=\"abt-citation\"><\/span><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Beta Karoten<\/h3>\n\n\n\n<p>\u03b2-karoten, yang merupakan prekursor dari vitamin A, dapat bereaksi dengan peroksil, hidroksil, dan superoksida radikal. Karotenoid memberikan efek antioksidan ketika konsentrasi oksigen rendah, dan akan berefek sebaliknya ketika berada pada konsentrasi oksigen yang tinggi. Selain itu, \u03b2-karoten mampu menghambat produksi TNF-\u03b1 sehingga produksi ROS\/RNS di dalam mitokondria juga menjadi terhambat. \u03b2-karoten juga memiliki potensi menyingkirkan radikal bebas lebih baik bila dibandingkan dengan vitamin E. <strong><span class=\"has-inline-color has-vivid-red-color\">Sumber makanan yang mengandung \u03b2-karoten diantaranya bayam, wortel, brokoli, labu kuning, ubi, dan mangga.<\/span><\/strong><span id=\"e83be65e-4ca2-40ed-ac01-9e3c5d29f330\" data-items=\"[&quot;785000060&quot;,&quot;1107249092&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2,4\u200b<\/sup><\/span> Terdapat penelitian yang menunjukkan rendahnya asupan \u03b2-karoten dan rendahnya kadar \u03b2-karoten dalam darah dapat mengindikasikan adanya resistensi insulin dan kejadian DM tipe 2.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Vitamin dan Diabetes Melitus<\/h2>\n\n\n\n<p><sup>\u200b<\/sup>Kemampuan vitamin C, vitamin E, dan \u03b2-karoten dalam menekan jumlah produksi oksidan serta melindungi komponen sel dari kerusakan, dapat melindungi tubuh dari kerusakan akibat oksidan yang berlebih\/kondisi stres oksidatif. Kapasitas antioksidan tubuh akan lebih mudah bekerja karena oksidan yang terbentuk tidak terlalu banyak. Selain itu, sel \u03b2 pankreas akan cenderung lebih aman dari kerusakan akibat adanya oksidan yang berlebih. Alasannya karena makanan tidak hanya terdiri dari satu macam zat gizi saja, sehingga lebih aman untuk pemenuhan zat gizi bagi tubuh.
