Orang dengan HIV AIDS (ODHA) membutuhkan pengaturan asupan makan yang sesuai sehingga risiko malnutrisi dapat diminimalisir. Mari kita simak lebih lanjut pada artikel berikut =)<\/p>\n\n\n\n\n\n<h2 class=\"wp-block-heading\">HIV AIDS<\/h2>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\"><em>Human Immunodeficiency Virus<\/em> (HIV) adalah virus yang menyerang bagian utama sistem kekebalan tubuh, yang menyebabkan tubuh terbuka terhadap serangan infeksi dan penyakit termasuk AIDS, yang merupakan tahap akhir dari penyakit tersebut.<\/span><\/strong> HIV merupakan retrovirus yang hanya memiliki seuntai RNA, sehingga virus HIV bereplikasi menggunakan suatu enzyme (reverse traskriptase) dengan memanfaatkan DNA yang dimiliki sel CD4+, sehingga DNA sel akan mentranskripsikan RNA virus selama replikasi.<span id=\"6cac7e57-ed6a-42f3-92a8-8d2b9d93d2c7\" data-items=\"[&quot;1945751700&quot;,&quot;3801044439&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b1,2\u200b<\/sup><\/span><\/p>\n\n\n\n<p id=\"tw-target-text\"><a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/hiv-aids\">HIV <\/a>tidak mudah ditularkan kecuali melalui pertukaran cairan selama kontak seksual, terinfeksi melalui transfusi darah, penggunaan jarum suntik bergantian, atau dari ibu yang terinfeksi HIV ke neonatus. Orang yang berisiko tinggi mengalami HIV termasuk laki-laki homoseksual dan biseksual, penderita hemofilia, pecandu narkoba, heteroseksual dengan banyak pasangan, dan bayi dari ibu HIV-positif. HIV mengganggu kemampuan respon imun yang sesuai. Malnutrisi dan komplikasinya lebih berdampak buruk bagi tubuh; HIV seringkali menyebabkan penurunan status gizi. Penurunan TEE dapat membantu menjaga massa lemak tubuh tanpa lemak dengan mengimbangi peningkatan <em>Resting Energy Expenditure<\/em>\/REE. <span id=\"7f24b2eb-e789-48f4-88d6-b3986deb922c\" data-items=\"[&quot;3990235101&quot;]\" contenteditable=\"false\" class=\"abt-citation\" data-has-children=\"true\"><sup>\u200b4\u200b<\/sup><\/span><\/p>\n\n\n\n<p id=\"tw-target-text\">Penurunan asupan kalori dibanding peningkatan REE, secara signifikan berkorelasi dengan penurunan berat badan. Orang yang terinfeksi HIV tanpa gejala, berat badannya stabil, dan tidak menggunakan <em>antiretroviral therapy<\/em> (ART), maka peningkatan asupan kalori mengkompensasi peningkatan REE dan dipercepat dengan pergantian protein.<span id=\"3cd4b1f9-5e14-4d40-9287-ed148003f826\" data-items=\"[&quot;3990235101&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b4\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Intervensi gizi pada ODHA dilakukan dengan tujuan : <span id=\"a82320dc-79ea-4de6-909e-3563bd4e3038\" data-items=\"[&quot;920369213&quot;]\" contenteditable=\"false\" class=\"abt-citation\" data-has-children=\"true\"><sup>\u200b3\u200b<\/sup><\/span><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Meningkatkan sistem imun utk mencegah infeksi oportunistik (kandidiasis oral, sirosis hepatik, dll)<\/li><li>Meningkatkan respon terhadap terapi (dg konseling gizi dan monev) <\/li><li>Menjaga berat badan (BB) 95-100% dari BB normal <\/li><li>Mengatasi demam dan gangguan gastrointestinal (mual, muntah, diare, konstipasi, dll) <\/li><li>Melakukan monitoring evaluasi terhadap penggunaan terapi alternatif <\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Kebutuhan Zat Gizi pada HIV AIDS<\/h2>\n\n\n\n<p>Pengaturan asupan makan merupakan hal yang penting diperhatikan pada orang dengan HIV AIDS. Mempertahankan pola makan yang sesuai dengan kondisi pasien; menggunakan <strong><span class=\"has-inline-color has-vivid-red-color\">diet tinggi energi tinggi protein<\/span><\/strong> dengan zat gizi yang cukup. Penambahan atau pemeliharaan berat badan dimungkinkan pada pasien dengan infeksi HIV dan stadium awal AIDS menggunakan suplemen.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Energi<\/h3>\n\n\n\n<p id=\"tw-target-text\">WHO merekomendasikan asupan energi di atas 10% dari energi yang diharapkan untuk infeksi HIV tanpa gejala. Dalam pedoman ESPEN untuk HIV dan penyakit menular, menyimpulkan bahwa tidak ada bukti kebutuhan energi pada infeksi HIV berbeda dari kelompok pasien lain sebagai studi tentang pengeluaran energi total belum menunjukkan hipermetabolisme. Namun, pada pasien HIV AIDS selama fase pemulihan setelah infeksi oportunistik kebutuhan energi meningkat 20 sampai 50%. Selain itu, fase anabolik setelah mulai ART cenderung meningkatkan kebutuhan energi sesuai persyaratan.<span id=\"226a808b-12c1-452d-a973-3547133f0de8\" data-items=\"[&quot;3990235101&quot;]\" contenteditable=\"false\" class=\"abt-citation\" data-has-children=\"true\"><sup>\u200b4\u200b<\/sup><\/span><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Protein<\/h3>\n\n\n\n<p><br>Pedoman ESPEN untuk terapi gizi pada HIV dan penyakit menular yaitu asupan protein 1,2 gram \/ kg BB \/ hari dalam fase stabil penyakit,  dapat ditingkatkan menjadi 1,5 gram \/ kg BB \/ hari selama sakit akut. Demam dan infeksi dapat semakin meningkatkan kebutuhan protein. Selain itu, peningkatan asupan energi juga diperlukan jika terjadi infeksi, demam, dan pneumonia.<span id=\"558f7f4b-2782-4821-9c2e-f788c6c18a45\" data-items=\"[&quot;3990235101&quot;]\" contenteditable=\"false\" class=\"abt-citation\" data-has-children=\"true\"><sup>\u200b4\u200b<\/sup><\/span><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Lemak<\/h3>\n\n\n\n<p>Menjaga asupan lemak pada orang dengan HIV AIDS sekitar 30 \u2013 35% dari total kalori. Jumlah dan jenis lemak yang dianjurkan berdasarkan kebutuhan energi, risiko kardiovaskular, dan kondisi peradangan Jika pasien berisiko mengalami penyakit kardiovaskular mungkin memerlukan asupan lemak yang lebih rendah dan rasio lemak tak jenuh yang lebih tinggi. Jumlah dan jenis karbohidrat direkomendasikan berdasarkan kebutuhan energi dan toleransi karbohidrat. Kondisi HIV AIDS dengan resistensi insulin dan <a href=\"https:\/\/ahligizi.id\/blog\/2019\/11\/23\/diet-diabetes-mellitus\/\">diabetes<\/a> memerlukan diet modifikasi untuk memodulasi kadar glukosa dan insulin. <span id=\"4ed5ec4d-589a-460d-b2b0-38bf16d79e34\" data-items=\"[&quot;1945751700&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b1\u200b<\/sup><\/span><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Serat<\/h3>\n\n\n\n<p id=\"tw-target-text\">Rekomendasi serat serupa dengan yang ada di individu yang sehat yaitu 14 gram \/ 1000 kkal. Pada kondisi mual, muntah, demam kebutuhan cairan ditambah, natrium, kalium, chlor perlu ditambah.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Mikronutrien<\/h3>\n\n\n\n<p>Suplemen multivitamin, mineral sebaiknya direkomendasikan namun tidak melebihi 100% dari AKG. WHO tidak merekomendasikan pemberian zat gizi mikro melebihi rekomendasi pada orang dengan HIV. Vitamin A dan beta karoten dapat memperbaiki permeabilitas GI dan diare. Defisiensi yg umumnya terjadi Vitamin A, B kompleks, C, E, selenium, Zink diajurkan pemenuhan zat gizi tersebut dari pemberian diet adekuat dengan variasi bahan makanan.
