Prevalensi Diabetes semakin meningkat tidak hanya di Indonesia namun juga di seluruh negara. Salah satu pilar Pengelolaan Diabetes Melitus yaitu Terapi Diet melalui Diet Rendah karbohidrat. Mari kita bahas pada artikel ini.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Penatalaksanaan Diabetes Mellitus <\/h2>\n\n\n\n<p>Diabetes Mellitus (DM) merupakan suatu kelompok penyakit metabolik dengan karakteristik hiperglikemia yang terjadi karena kelainan sekresi insulin, kerja insulin, atau keduanya. Resistensi insulin pada sel otot dan hati, serta kegagalan sel beta pankreas telah dikenal sebagai patofisiologi kerusakan sentral dari DM tipe 2. <strong><span class=\"has-inline-color has-vivid-red-color\">Diagnosis DM ditegakkan atas dasar pemeriksaan kadar glukosa darah<\/span><\/strong>. <span id=\"10f5ee00-9d8e-46a1-bf46-635517b9b28d\" data-items=\"[&quot;2535459112&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b1\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Penatalaksanan DM dimulai dengan menerapkan pola hidup sehat (terapi diet dan aktivitas fisik) bersamaan dengan intervensi farmakologis dengan obat anti hiperglikemia oral dan atau suntikan. Pengetahuan tentang pemantauan mandiri, tanda, dan gejala hipoglikemia dab cara mengatasinya harus diberikan kepada pasien. <strong><span class=\"has-inline-color has-vivid-red-color\">Terdapat 5 pilar penanganan DM yaitu edukasi, terapi diet, latihan fisik, terapi farmakologis, serta monitoring kadar glukosa darah mandiri<\/span><\/strong>. <span id=\"adfd1159-230a-4fb1-a6c7-cdea8c63ab3b\" data-items=\"[&quot;2535459112&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b1\u200b<\/sup><\/span><\/p>\n\n\n\n<p>BACA : <a href=\"https:\/\/ahligizi.id\/blog\/2019\/11\/23\/diet-diabetes-mellitus\/\">Diet Diabetes Mellitus<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Diet Rendah Karbohidrat<\/h2>\n\n\n\n<p id=\"tw-target-text\"><strong><span class=\"has-inline-color has-vivid-red-color\">Terapi diet merupakan salah satu strategi untuk mengatasi penyakit diabetes mellitus (DM) karena dapat menurunkan beban sel beta pankreas, serta memperbaiki kadar glukosa darah, profil lipid, dan status kognitif<\/span><\/strong>. Namun, kepatuhan yang baik terhadap diet diabetes adalah dasar dari terapi diet. Asupan makan yang dianjurkan pada studi di China terdiri dari 500 gram sayur, 100 gram makanan pokok (setara 300 gram karbohidrat), 3 sendok makan minyak\/hari, konsumsi buah dengan syarat (glikemik stabil, buah dengan indek glikemik rendah, diantara waktu makan, buah mengandung kalori yang menambah total energi), konsumsi protein (1 gelas susu, 1 butir telur, 50 gram ikan\/udang, 50 gram kedelai\/produk olahannya, 50 gram daging), serta 6 gram garam\/hari.<span id=\"888effb2-783f-469a-b8de-327cef857e62\" data-items=\"[&quot;15610170&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2\u200b<\/sup><\/span> Diet rendah karbohidrat terbukti efektif pada obesitas, menurunkan berat badan secara signifikan, serta efektif memperbaiki profil lipid darah dan resistensi insulin. Dalam beberapa tahun terakhir, <strong><span class=\"has-inline-color has-vivid-red-color\"><em>American Diabetes Association<\/em> (ADA) dan Diabetes UK  menegaskan keefektifan diet rendah karbohidrat dalam mengurangi berat badan, meningkatkan glukosa darah, dan mengatur profil lipid pada pasien DM<\/span><\/strong>.<span id=\"7480b0a5-55df-4c58-bf78-063ee579b787\" data-items=\"[&quot;1391958302&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b3\u200b<\/sup><\/span><\/p>\n\n\n\n<p>BACA LEBIH LANJUT : <a href=\"https:\/\/ahligizi.id\/blog\/2020\/08\/17\/pengaturan-indeks-glikemik-beban-glikemik-dan-penghitungan-karbohidrat-pada-diabetes-mellitus\/\">Pengaturan Indeks Glikemik, Beban Glikemik, dan Peghitungan Karbohidrat pada Diabetes Mellitus<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Profil Glukosa Darah pada Diet Rendah Karbohidrat<\/h2>\n\n\n\n<p id=\"tw-target-text\">Kadar HbA1c, kadar glukosa darah puasa, dan glukosa darah 2 jam postprandial yang tinggi adalah beberapa tantangan yang dihadapi pasien dengan DMT2 dan parameter ini dapat digunakan sebagai indikator utama menetapkan kontrol glikemik. Kadar HbA1c dapat mencerminkan kadar glukosa darah dalam 2 ~ 3 bulan sebelum dan dalam jangka panjang ekstraksi darah kontrol glikemik pasien. <strong><span class=\"has-inline-color has-vivid-red-color\">Hasil penelitian menunjukkan bahwa kadar HbA1c pada diet rendah karbohidrat (8,5%) menurun secara signifikan dibandingkan dengan diet rendah lemak (4%)<\/span><\/strong>.<span id=\"4c5ac0bf-e350-4c9d-a858-9963750ae1c6\" data-items=\"[&quot;15610170&quot;,&quot;1391958302&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2,3\u200b<\/sup><\/span> Alasannya  karena penurunan tingkat makanan indeks glikemik tinggi, jumlah total makanan tinggi karbohidrat, dan peningkatan asupan kacang-kacangan, yang dapat membantu memperbaiki hiperglikemia dan sensitivitas insulin.<span id=\"0375e3da-c2cf-4715-b00a-67173d884af4\" data-items=\"[&quot;1751003526&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b4\u200b<\/sup><\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Profil Lemak pada Diet Rendah Karbohidrat<\/h2>\n\n\n\n<p id=\"tw-target-text\">Kacang tergolong makanan tinggi lemak tak jenuh dengan energi tinggi, namun tidak meningkatkan berat badan pasien karena meningkatkan rasa kenyang dan menyebabkan efek kompensasi makanan yang kua. Selain itu, efisiensi penyerapan energi kacang-kacangan rendah dan energi total tidak meningkat. Studi ini menunjukkan bahwa<strong><span class=\"has-inline-color has-vivid-red-color\"> indeks massa tubuh (IMT) pada kelompok diet rendah karbohidrat menurun<\/span><\/strong>.<span id=\"c1982dc7-27e5-423e-8c93-9af7502a4e52\" data-items=\"[&quot;15610170&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2\u200b<\/sup><\/span> Diabetes secara signifikan berhubungan dengan dislipidemia. Lovejoy dkk. menemukan bahwa kadar total kolesterol darah dalam makanan diperkaya dengan almond lebih rendah sebesar 21%. <span id=\"22721f23-fa1d-4606-9531-a08c77d16905\" data-items=\"[&quot;1876580179&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b5\u200b<\/sup><\/span> <strong><span class=\"has-inline-color has-vivid-red-color\">Studi menemukan bahwa kadar total kolesterol darah menurun secara signifikan sebesar 7,4% pada kelompok diet rendah karbohidrat<\/span><\/strong>.
