Tubuh membutuhkan lebih banyak air setiap hari dibanding zat gizi lainnya. Kebutuhan air selama kehamilan mengalami peningkatan. Berapa kebutuhan air pada ibu hamil yang direkomendasikan?<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Perubahan Komposisi Cairan Selama Kehamilan<\/h2>\n\n\n\n<p id=\"tw-target-text\">Kehamilan adalah keadaan anabolik yang mempengaruhi jaringan ibu terkait sintesis hormon untuk mendukung kehamilan yang baik. Progesteron menginduksi penumpukan lemak untuk mengisolasi bayi, mendukung cadangan energi, dan merelaksasi otot polos, yang akan menyebabkan penurunan motilitas usus untuk meningkatkan absorpsi zat gizi. <strong><span class=\"has-inline-color has-vivid-red-color\">Estrogen meningkat pesat selama kehamilan untuk mendorong pertumbuhan, fungsi rahim, dan retensi air<\/span><\/strong>. Sekitar dua pertiga cairan intraseluler (darah dan jaringan tubuh) dan sepertiganya cairan ekstraseluler (cairan dalam ruang antar sel). Volume plasma mulai meningkat dalam beberapa minggu setelah konsepsi dan mencapai maksimum pada 34 minggu. Lonjakan volume plasma selama awal kehamilan menjadi alasan utama ibu hamil mudah lelah. Kelelahan terkait dengan peningkatan volume plasma terjadi di bulan kedua dan pada bulan ketiga kehamilan menurun sebagai kompensasi dari penyesuaian fisiologis.<span id=\"e1875b5d-cf24-41d0-8798-c145c38f02c2\" data-items=\"[&quot;1159089220&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2\u200b<\/sup><\/span><\/p>\n\n\n\n<p id=\"tw-target-text\">Peningkatan cairan tubuh sangat bervariasi di antara wanita selama kehamilan normal. Tingginya cairan dikaitkan dengan peningkatan derajat edema dan penambahan berat badan. Jika tidak hipertensi, umumnya edema mencerminkan perluasan volume plasma yang baik. Berat lahir sangat terkait ke volume plasma: umumnya, semakin besar ekspansi, semakin besar ukuran bayi baru lahir. Peningkatan volume air dalam darah bertanggung jawab atas &#8220;efek pengenceran&#8221; kehamilan pada konsentrasi darah dari beberapa vitamin dan mineral. Kadar vitamin larut lemak dalam darah cenderung meningkat pada kehamilan, sedangkan kadar vitamin larut dalam air vitamin cenderung menurun.<span id=\"591ae3c9-7f6e-4b56-9f67-4ef2755d725e\" data-items=\"[&quot;1159089220&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2\u200b<\/sup><\/span> <\/p>\n\n\n\n<pre class=\"wp-block-preformatted\">BACA LEBIH LANJUT : <a href=\"https:\/\/ahligizi.id\/blog\/2019\/11\/22\/diet-pada-ibu-hamil\/\">Diet pada Ibu Hamil<\/a><\/pre>\n\n\n\n<h2 class=\"wp-block-heading\">Kebutuhan Cairan pada Ibu Hamil<\/h2>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">Peningkatan kebutuhan air selama kehamilan secara umum dipenuhi dengan peningkatan rasa haus. Rata-rata wanita mengkonsumsi sekitar 9 cangkir cairan setiap hari selama kehamilan<\/span><\/strong>. Ibu hamil yang melakukan aktivitas fisik di iklim panas dan lembab harus minum secukupnya untuk menjaga urine berwarna cerah dan normal dalam volume. Air, jus buah yang diencerkan, es teh, dan minuman tanpa <a href=\"https:\/\/ahligizi.id\/blog\/2020\/10\/12\/konsumsi-gula-dan-pemanis\/\">pemanis<\/a> lainnya merupakan pilihan yang baik untuk menjaga status hidrasi tetap baik.<span id=\"ddf03165-b8b3-43bf-84d6-c08c5d4ba308\" data-items=\"[&quot;1159089220&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2\u200b<\/sup><\/span><\/p>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">Menurut Angka Kecukupan Gizi (AKG) Indonesia tahun 2019 pada trimester 1 &#8211; 3 kehamilan kebutuhan cairan meningkat 300 ml\/hari sehingga kebutuhan cairan pada ibu hamil sekitar 2,65 L\/hari<\/span><\/strong>.<span id=\"9040df3d-49a5-47df-937e-8971ab64a0d7\" data-items=\"[&quot;558832490&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b3\u200b<\/sup><\/span> Kebutuhan cairan pada ibu hamil di Amerika Serikat yaitu 3 L\/hari sedangkan di Korea kebutuhan cairan pada ibu hamil sebesar 2,2 &#8211; 2,3 L\/hari, dan di Filipina menerapkan rekomendasi 1 ml\/kkal ditambah 300 ml.<span id=\"88faa638-dd3f-48f2-855f-1c7c645d3403\" data-items=\"[&quot;1233519302&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b4\u200b<\/sup><\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Oligohidramnion<\/h2>\n\n\n\n<p>Salah satu gangguan dalam kehamilan adalah oligohidramnion, yang sangat berkaitan dengan mortalitas dan morbiditas perinatal yang tinggi. Cairan amnion merupakan cairan kompleks yang berfungsi penting bagi kesehatan janin. <strong><span class=\"has-inline-color has-vivid-red-color\">Pemeliharaan hidrasi ibu jangka panjang dengan asupan air yang cukup dapat meningkatkan volume cairan amnion serta mencegah oligohidramnion<\/span><\/strong>.<span id=\"15919bf2-2eac-4d57-878b-86a2e18203fb\" data-items=\"[&quot;2161308090&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b5\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Namun demikian, pemberian cairan yang banyak tidak serta merta mencegah oligohidramnion. Kondisi ini terjadi akibat kebocoran cairan amnion atau penurunan produksi urin janin. Jumlah produksi cairan amnion telah diatur sehingga cukup pada kehamilan normal. Penyebab oligohidramnion antara lain gangguan kromosom janin, kelainan ginjal kongenital, gagal tumbuh, kehamilan lewat waktu, hipertensi, pre eklampsia, diabetes, dan hiposia kronik pada ibu hamil.<span id=\"009dd756-7bc5-4ee3-b512-b4bd42e7c97d\" data-items=\"[&quot;45259208&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b6\u200b<\/sup><\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Konstipasi dan Hemoroid<\/h2>\n\n\n\n<p id=\"tw-target-text\"><strong><span class=\"has-inline-color has-vivid-red-color\">Ibu hamil berisiko mengalami konstipasi jika  asupan air dan serat kurang<\/span><\/strong>. Wanita yang dirawat dengan ondansetron karena mual dan muntah sering mengalami konstipasi yang parah. Kompresi dasar panggul oleh janin, juga mengejan saat buang air besar, meningkatkan risiko hemoroid\/wasir. Peningkatan konsumsi cairan dan makanan kaya serat seperti buah dan sayur dapat mengontrol masalah ini.<span id=\"c615b499-a4d9-4eea-95ce-e4c6cd814cb2\" data-items=\"[&quot;1233519302&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b4\u200b<\/sup><\/span> <\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Edema dan Kram Kaki<\/h2>\n\n\n\n<p id=\"tw-target-text\">Edema\/bengkak fisiologis ringan biasanya muncul pada trimester ketiga. Edema normal di ekstremitas bawah (kaki) disebabkan tekanan pembesaran rahim di pembuluh vena cava, gangguan kembalinya aliran darah ke jantung. Ketika ibu hamil bersandar,  efek mekanik dihilangkan, dan cairan ekstravaskular dimobilisasi dan akhirnya dieliminasi dengan peningkatan urin. Tidak ada  intervensi diet yang diperlukan jika asupan protein cukup. Namun, <strong><span class=\"has-inline-color has-vivid-red-color\">jika urin berwarna gelap dan \/ atau mengalami pembengkakan tangan, peningkatan asupan cairan dianjurkan<\/span><\/strong>. <strong><span class=\"has-inline-color has-vivid-red-color\">Peningkatan asupan cairan juga dianjurkan untuk kram kaki<\/span><\/strong>. Ibu hamil  disarankan untuk melakukan peregangan jari-jari kaki.<span id=\"9be63186-b611-43e9-9807-3a977d22d661\" data-items=\"[&quot;1233519302&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b4\u200b<\/sup><\/span> <\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Hiperemesis Gravidarum<\/h2>\n\n\n\n<p>adalah rasa mual dan muntah yang berlebihan dalam waktu relatif lama yang terjadi pada awal kehamilan (hingga trimester kedua). Bila kondisi ini tidak diatasi dapat menyebabkan hipovolemia dan penurunan berat badan.<span id=\"1c91d4f8-b733-40e5-a291-6aa0d1e24f30\" data-items=\"[&quot;2387712018&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b7\u200b<\/sup><\/span> Banyak terapi terkait mual muntah pada ibu hamil namun intervensi diet merupakan terapi yang aman dan efektif dibanding terapi lain. <strong><span class=\"has-inline-color has-vivid-red-color\">Konsumsi telur rebus, keripik kentang, popcorn, yogurt, krakers, dan makanan tinggi karbohidrat dapat ditoleransi dengan baik oleh ibu hamil yang mengalami mual dan muntah<\/span><\/strong>. Ibu hamil dengan hiperemesis membutuhkan terapi rehidrasi untuk mengembalikan keseimbangan cairan dan elektrolit, biasanya dibantu melalui cairan intravena. <span id=\"c471bee4-070d-4468-bb1c-44dbe860f932\" data-items=\"[&quot;1159089220&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2\u200b<\/sup><\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Pre-eklampsia<\/h2>\n\n\n\n<p>ditandai oleh hipertensi, proteinuria, dan edema. Bila tidak diobati dapat menimbulkan kejang, koma, bahkan kematian. Asupan cairan pada kondisi pre-eklampsia sangat penting. Status hidrasi ibu hamil perlu terus dimonitor. Cairan diberikan maksimal 2,5 L\/hari dan pada oliguria cairan dibatasi serta disesuaikan dengan cairan yang keluar melalui urin, muntah, serta kehilangan cairan yang tidak disadari (keringat dan pernafasan).
