Mari kita simak pada artikel ini. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Talasemia<\/h2>\n\n\n\n<p>Talasemia merupakan penyakit kelainan darah merah yang diturunkan dari kedua orangtua kepada anak dan keturunannya. Penyakit ini terjadi karena berkurangnya atau tidak terbentuknya protein pembentuk hemoglobin utama manusia. Hal ini menyebabkan eritrosit mudah pecah dan menyebabkan pasien menjadi pucat karena kekurangan darah.<span id=\"b5f0cb08-10bf-4f67-b050-339099bf12a7\" data-items=\"[&quot;3245204405&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b1\u200b<\/sup><\/span> Pasien talasemia memerlukan tranfusi darah seumur hidup.<sup> <\/sup><em>Thalassemia International Federation<\/em> (TIF) melaporkan diseluruh dunia terdaftar sekitar 200.000 pasien yang hidup dan mendapatkan terapi. Pembawa sifat talasemia di seluruh dunia diperkirakan sebanyak 1,5% atau 80-90%.<span id=\"8d1b002c-fd37-4546-9991-8e5af040ebde\" data-items=\"[&quot;1663228175&quot;,&quot;312559367&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2,3\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Pada penyakit talasemia pengaturan diet yang optimal berperan sebagai modalitas dalam pengobatan jangka panjang dan untuk mencegah gangguan gizi, gangguan pertumbuhan, gangguan perkembangan, dan defisiensi imun yang mungkin berhubungan dengan malnutrisi sekunder.<span id=\"34e47db7-c28c-44d8-94e3-d6fde619e17c\" data-items=\"[&quot;1811053654&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b4\u200b<\/sup><\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Kategori Talasemia<\/h2>\n\n\n\n<p>Talasemia dibagi menjadi 3 jenis berdasarkan gejalanya.<span id=\"a8ca39de-c394-46d0-8e24-1b32b4e26ff5\" data-items=\"[&quot;1663228175&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2\u200b<\/sup><\/span><span id=\"3b95d273-a924-4fee-a6dc-172a47eb9ab8\" data-items=\"[&quot;396025557&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b5\u200b<\/sup><\/span><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Talasemia mayor<\/li><\/ul>\n\n\n\n<p id=\"tw-target-text\">Talasemia mayor menyebabkan anemia hemolitik, pertumbuhan yang buruk, dan kelainan tulang selama masa bayi. Gejala dapat dimulai sejak usia 3 bulan. Dalam satu atau dua tahun pertama kehidupan dan dengan tidak adanya transfusi, seorang anak dapat menunjukkan anemia parah dan pembesaran wajah dan lainnya tulang. Anak-anak ini mungkin pucat atau sakit kuning, memiliki yang miskin nafsu makan, gagal tumbuh secara normal, dan memiliki limpa yang membesar, hati, atau hati. Insiden batu empedu sangat umum dalam populasi ini. Anak-anak yang terkena akan membutuhkan perawatan rutin transfusi darah seumur hidup<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Talasemia intermedia<\/li><\/ul>\n\n\n\n<p>Biasanya baru terdiagnosis pada anak yang lebih besar, dan biasanya tidak membutuhkan transfusi darah rutin. Talasemia ini menyebabkan anemia hemolitik, sel eritrosit yang mudah rusak dan abnormal hemoglobin.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Talasemia minor\/trait\/pembawa sifat<\/li><\/ul>\n\n\n\n<p>Biasanya tidak bergejala, tampak normal, namun pada pemeriksaan darah dapat ditemukan kadar hemoglobin yang sedikit di bawah normal. Beberapa gejala yang dialami oleh penderita talasemia adalah sesak nafas, pucat, denyut jantung jadi cepat, pusing berputar, dan perubahan bentuk kepala.<\/p>\n\n\n\n<p>Jika seorang talasemia mayor menikah dengan orang normal, probabilitas anaknya 100% pembawa sifat. Jika seorang talasemia minor menikah dengan orang normal, probabilitas anaknya sehat 50% dan menjadi talasemia minor 50%. Hal ini bertujuan untuk mempunyai keuntungan klinis yang nyata, memungkinkan aktivitas yang normal dengan nyaman, mencegah ekspansi sumsum tulang dan masalah kosmetik yang berkaitan dengan perubahan tulang muka, meminimalkan dilatasi jantung dan osteoporosis.<span id=\"7fa69494-1878-4deb-afa8-a095d67ecd9d\" data-items=\"[&quot;435734851&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b6\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Penatalaksanaan talasemia yang dapat dilakukan antara lain:<span id=\"b40ec979-4a03-4549-a612-496fe3e81db9\" data-items=\"[&quot;3815015949&quot;]\" contenteditable=\"false\" class=\"abt-citation\" data-has-children=\"true\"><sup>\u200b7\u200b<\/sup><\/span><\/p>\n\n\n\n<ol class=\"wp-block-list\"><li> Transfusi darah<\/li><\/ol>\n\n\n\n<p>Karena sel darah merah pasien cepat hancur, perlu dilakukan transfusi darah untuk menggantikan sel darah merah yang rusak. Transfusi darah dan peningkatan absorbsi zat besi mengakibatkan kelebihan zat besi dan kerusakan jaringan. Kelebihan zat besi terakumulasi, menyebabkan kerusakan hati, jantung, dan hipofisis dan kegagalan organ-organ tersebut. Komplikasi jantung yang disebabkan oleh deposisi zat besi, seperti kardiomiopati, adalah penyebab utama kematian.<\/p>\n\n\n\n<p>2. Terapi kelasi besi<\/p>\n\n\n\n<p>akibat transfusi darah jangka panjang akan menyebabkan kelebihan zat besi karena setiap 500 mL darah membawa kira-kira 200 mg besi ke jaringan yang tidak dapat diekskresikan yang mengakibatkan terjadi penumpukan zat besi dalam jaringan tubuh sehingga diperlukan pemberian obat pengkelasi besi untuk membuang zat besi yang menumpuk di organ-organ&nbsp;tubuh. <\/p>\n\n\n\n<p>3. Pemberian asam folat<\/p>\n\n\n\n<p>diperlukan untuk memebuhi kebutuhan pasien talasemia, khususnya pada pasien yang jarang menerima transfusi darah untuk mencegah terjadinya komplikasi.<\/p>\n\n\n\n<p>4. Transplantasi sumsum tulang<\/p>\n\n\n\n<p id=\"tw-target-text\">dapat menjadi kuratif untuk beberapa anak dengan talasemia beta mayor. Darah tali pusat adalah darah yang tertinggal di pusar dan plasenta setelah lahir;  sumber yang kaya sel yang berkembang biak menjadi eritrosit untuk sistem kekebalan tubuh. Transplantasi stem cell menawarkan kesempatan bagi individu dengan talasemia untuk menjalani hidup yang lebih normal.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Diet Talasemia<\/h2>\n\n\n\n<p>Pasien talasemia membutuhkan diet talasemia berupa pengaturan makronutrien dan mikronutrien.  <strong><span class=\"has-inline-color has-vivid-red-color\">Diet tinggi protein, energi, vitamin B kompleks berkualitas tinggi (terutama asam folat dan vitamin B12), dan seng bermanfaat. Untuk mencegah kelebihan zat besi, hindari penggunaan multivitamin \u2013 suplemen mineral yang mengandung zat besi dan <a href=\"https:\/\/ahligizi.id\/blog\/2020\/08\/04\/konsumsi-vitamin-c-aman-saat-pandemi\/\">vitamin C<\/a> dalam jumlah banyak.<\/span><\/strong> <strong><span class=\"has-inline-color has-vivid-red-color\">Berikan asupan cairan yang adekuat.<\/span><\/strong> Jika ada hiperglikemia dan diabetes, <a href=\"https:\/\/ahligizi.id\/blog\/2020\/08\/17\/pengaturan-indeks-glikemik-beban-glikemik-dan-penghitungan-karbohidrat-pada-diabetes-mellitus\/\">gunakan carbohydrate counting untuk mengontrol kadar glukosa<\/a>. Mikronutrien yang dibutuhkan antara lain:<span id=\"776f35fa-ddaf-4193-b1d2-d563869d7059\" data-items=\"[&quot;1663228175&quot;,&quot;1811053654&quot;,&quot;396025557&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2,4,5\u200b<\/sup><\/span><\/p>\n\n\n\n<ul class=\"wp-block-list\" type=\"a\"><li>Kalsium (1 g\/hari)<\/li><\/ul>\n\n\n\n<p>Pada penderita talasemia yang tidak mendapatkan transfusi darah secara cukup terjadi peningkatan aktivitas sumsum tulang, sehingga korteks tulang menjadi tipis. Sebaliknya, pemberian<strong><span class=\"has-inline-color has-vivid-red-color\"> transfusi berulang akan menyebabkan terjadinya hemosiderosis<\/span><\/strong>. Contoh makanan tinggi kalsium: keju, susu, wortel, biji-bijian dan kacang-kacangan.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Seng (suplementasi dosis tinggi 45 mg\/hari)<\/li><\/ul>\n\n\n\n<p>Sebagian seng menggunakan transferin sebagai alat transport besi.&nbsp;Kekurangan seng yang berat pada penderita talasemia menyebabkan gangguan pertumbuhan, defisiensi imun, serta hambatan pada proses penyembuhan luka. Contoh makanan tinggi seng: ikan laut dan susu.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Vitamin D<\/li><\/ul>\n\n\n\n<p>Membantu pembentukan dan pemeliharaan tulang dengan cara mengatur agar kalsium dan fosfat tersedia di dalam darah untuk diendapkan pada proses pengerasan tulang. Contoh makanan yang mengandung vitamin D: minyak ikan, kacang &#8211; kacangan, susu.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Vitamin E (200-400 IU\/hari)<\/li><\/ul>\n\n\n\n<p>Sebagai antioksidan, sehingga dapat melindungi sel dari efek toksisitas besi. Contoh makanan yang mengandung vitamin E: kecambah, kacang-kacangan, dan gandum.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Asam Folat (1 mg\/hari)<\/li><\/ul>\n\n\n\n<p>Pada pasien talasemia yang tidak mendapat transfusi secara cukup biasanya terjadi defisiensi asam folat. Contoh makanan yang mengandung asam folat: sayur berdaun hijau, kacang-kacangan.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Zat Besi (&lt;10 th: maks 10 mg\/hari; &gt;10 th: maks 18 mg\/hari)<\/li><\/ul>\n\n\n\n<p>Talasemia berpotensi mengalami hemosiderosis. Oleh karena itu, makanan yang kadar kandungan zat besinya tinggi harus dibatasi.&nbsp;Contoh makanan tinggi besi: daging merah, hati, telur, kerang.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><a href=\"https:\/\/ahligizi.id\/blog\/2020\/08\/04\/konsumsi-vitamin-c-aman-saat-pandemi\/\">Vitamin C<\/a> (100-250 mg\/hari)<\/li><\/ul>\n\n\n\n<p>Membantu penyerapan besi &amp; meningkatkan kadar besi yang aktif di dalam sel.
