Pada artikel sebelumnya kita telah membahas terkait <a href=\"https:\/\/ahligizi.id\/blog\/2019\/12\/12\/diet-batu-ginjal\/\">diet pada penyakit batu ginjal<\/a>. Sekarang ini ada pedoman gizi terbaru untuk penyakit ginjal. Mari kita simak <\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><em>Kidney Disease Outcomes Quality Initiative (KDOQI)<\/em> 2020<\/h2>\n\n\n\n<p><em>The National Kidney Foundation\u2019s KDOQI<\/em> memberikan pedoman gizi pada penyakit ginjal <strong>berbasis fakta (<em>evidence-based<\/em>) <\/strong>sejak 1999. Pedoman gizi tersebut mencakup pasien dengan penyakit ginjal stadium akhir (Gagal ginjal kronik\/GGK) lanjut, tetapi juga pasien GGK dengan stadium 1- 5 yang tidak menerima dialisis dan pasien dengan transplantasi ginjal. <\/p>\n\n\n\n<p>Pernyataan pedoman yang diperbarui pada 6 bidang : asesmen gizi, terapi gizi klinik, protein dan asupan energi, suplementasi gizi, mikronutrien, dan elektrolit. Pedoman tersebut terutama mencakup pengelolaan makanan dari semua kemungkinan intervensi gizi.<span id=\"39cf2032-87f8-455d-9110-5f79e891787d\" data-items=\"[&quot;1498557955&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b1\u200b<\/sup><\/span><\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"612\" height=\"266\" src=\"https:\/\/ahligizi.id\/blog\/wp-content\/uploads\/2020\/08\/image-16.png\" alt=\"\" class=\"wp-image-1046\" srcset=\"https:\/\/ahligizi.id\/blog\/wp-content\/uploads\/2020\/08\/image-16.png 612w, https:\/\/ahligizi.id\/blog\/wp-content\/uploads\/2020\/08\/image-16-300x130.png 300w\" sizes=\"auto, (max-width: 612px) 100vw, 612px\" \/><\/figure><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Rangkuman Pedoman Gizi untuk Penyakit Ginjal<\/h2>\n\n\n\n<p><strong>Berikut rangkuman singkat pedoman gizi untuk GGK menurut KDOQI <\/strong><span id=\"2fabd737-74d8-4c32-af95-3d2754454998\" data-items=\"[&quot;1498557955&quot;]\" contenteditable=\"false\" class=\"abt-citation\" data-has-children=\"true\"><sup>\u200b1\u200b<\/sup><\/span><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Asesmen Gizi<\/h3>\n\n\n\n<ul class=\"wp-block-list\"><li>Pada orang dewasa GGK stadium 5 dengan <strong><span class=\"has-inline-color has-vivid-red-color\">Peritoneal dialisis (PD)<\/span><\/strong>,<strong><span class=\"has-inline-color has-vivid-red-color\"> status berat badan kurang<\/span><\/strong> (berdasarkan indeks massa tubuh\/IMT) dapat digunakan sebagai <strong><span class=\"has-inline-color has-vivid-red-color\">prediktor mortalitas<\/span><\/strong> <\/li><li>Pada orang dewasa GGK stadium 5 dengan <strong><span class=\"has-inline-color has-vivid-red-color\">hemodialisis (HD)<\/span><\/strong>,  status kelebihan berat badan atau obesitas dapat digunakan sebagai prediktor mortalitas yang lebih rendah, sedangkan, <strong><span class=\"has-inline-color has-vivid-red-color\">status berat badan kurang dan obesitas morbid <\/span><\/strong>dapat digunakan sebagai <strong><span class=\"has-inline-color has-vivid-red-color\">prediktor mortalitas<\/span><\/strong> <\/li><li>Dewasa GGK stadium 5 dengan hemodialisis (HD), <strong><span class=\"has-inline-color has-vivid-red-color\">serum albumin<\/span><\/strong> dapat digunakan sebagai <strong><span class=\"has-inline-color has-vivid-red-color\">prediktor rawat inap dan mortalitas<\/span><\/strong><\/li><li>Pada orang dewasa GGK stadium 1-5 , <strong><span class=\"has-inline-color has-vivid-red-color\">kekuatan pegangan tangan (<em>handgrip strength<\/em>)  <\/span><\/strong>dapat digunakan sebagai <strong><span class=\"has-inline-color has-vivid-red-color\">indikator status energi protein <\/span><\/strong>dan fungsi status ketika pengukuran sebelumnya tersedia untuk perbandingan <\/li><li>Pasien GGK stadium 5, penggunaan <strong><span class=\"has-inline-color has-vivid-red-color\">S<em>ubjective Global Assessment<\/em> (SGA) <\/span><\/strong>sebagai <strong><span class=\"has-inline-color has-vivid-red-color\">alat yang valid dan andal untuk menilai status gizi<\/span><\/strong> <\/li><\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Terapi Gizi <\/h3>\n\n\n\n<p id=\"tw-target-text\">Pasien GGK stadium 1-5, dietisien\/nutrisionis berkolaborasi dengan dokter, perawat, dan tenaga medias lain untuk terapi gizi. <\/p>\n\n\n\n<p id=\"tw-target-text\">Tujuan terapi gizi adalah untuk mengoptimalkan status gizi, dan untuk meminimalkan risiko yang ditimbulkan oleh kondisi komorbiditas dan perubahan metabolisme pada perkembangan penyakit ginjal dan hasil klinis yang merugikan.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Kebutuhan Protein dan Energi<\/h3>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">Pasien GGK stadium 3-5 tanpa dialisis dan diabetes mellitus (DM)<\/span><\/strong> dengan kondisi metabolis stabil direkomendasikan pembatasan protein untuk menurunkan risiko GGK tahap akhir dan memperbaiki kualitas hidup pasien<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong><span class=\"has-inline-color has-vivid-red-color\">Diet Rendah Protein sebanyak 0,55 &#8211; 0,6 g\/kgBB\/hari<\/span><\/strong><\/li><li>Diet Protein Sangat Rendah 0,28 &#8211; 0,43 g\/kgBB\/hari dengan tambahan analog asam keto \/ asam amino<\/li><\/ul>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">Pasien dewasa GGK stadium 3 &#8211; 5 tanpa dialisis dan dengan DM<\/span><\/strong> = <strong><span class=\"has-inline-color has-vivid-red-color\">asupan protein 0,6 &#8211; 0,8 g\/kgBB\/hari <\/span><\/strong>untuk menjaga status gizi dan kontrol glikemik yang baik<\/p>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">GGK stadium 5 dengan dialisis (HD\/PD) tanpa DM <\/span><\/strong>dengan kondisi metabolis stabil direkomendasikan <strong><span class=\"has-inline-color has-vivid-red-color\">asupan protein 1,0 &#8211; 1,2 g\/kgBB\/hari<\/span><\/strong> untuk menjaga status gizi baik<\/p>\n\n\n\n<p>Pengidap<strong><span class=\"has-inline-color has-vivid-red-color\"> GGK stadium 5 dengan dialisis (HD\/PD) dan DM<\/span><\/strong> dengan kondisi metabolis stabil direkomendasikan <strong><span class=\"has-inline-color has-vivid-red-color\">asupan protein 1,0 &#8211; 1,2 g\/kgBB\/hari <\/span><\/strong>untuk menjaga status gizi baik. Untuk pasien dengan risiko hiper\/hipoglikemia, asupan protein yang lebih tinggi dapat diberikan untuk menjaga kontrol glikemik. <\/p>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">Pasien GGK stadium 1-5 atau post transplantasi<\/span><\/strong> dengan metabolis stabil direkomendasikan<strong><span class=\"has-inline-color has-vivid-red-color\"> asupan energi 25 &#8211; 35 kkal\/kgBB\/hari <\/span><\/strong>berdasar usia, jenis kelamin, tingkat aktivitas fisik, komposisi tubuh, berat badan, stadium GGK, dan ada tidaknya inflamasi untuk menjaga status gizi normal <\/p>\n\n\n\n<p><strong><span class=\"has-inline-color has-vivid-red-color\">Tidak ada cukup bukti untuk merekomendasikan jenis protein tertentu (nabati vs hewani)<\/span><\/strong> dalam hal pengaruhnya terhadap status gizi, kadar kalsium atau fosfor, atau profil lipid.<\/p>\n\n\n\n<p>Pasien GGK stadium 1-5 yang tidak menjalani dialisis atau pasca transplantasi, dengan atau tanpa dislipidemia, disarankan <strong><span class=\"has-inline-color has-vivid-red-color\">Diet Mediterania<\/span><\/strong> untuk meningkatkan profil lipid<\/p>\n\n\n\n<p>Pasien GGK stadium 1-4 disarankan <strong><span class=\"has-inline-color has-vivid-red-color\">meningkatkan asupan sayur dan buah<\/span><\/strong> untuk menurunkan berat badan, tekanan darah, dan <em>net acid production <\/em>(NEAP)<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Suplementasi Gizi<\/h3>\n\n\n\n<p id=\"tw-target-text\">Pada orang dewasa GGK stadium 3-5atau pasca transplantasiyang berisiko kekurangan energi protein, disarankan percobaan suplemen gizi oral selama 3 bulan untuk memperbaiki status gizi, jika hanya melalui konseling diet saja tidak mencapai asupan energi dan protein yang cukup untuk memenuhi kebutuhan gizi<\/p>\n\n\n\n<p>Pasien GGK stadium 5 dengan HD\/PD direkomendasikan <strong><span class=\"has-inline-color has-vivid-red-color\">asupan PUFA 1,34 g\/hari<\/span><\/strong> untuk menurunkan kadar trigliserid dan LDL serta meningkatkan kadar HDL<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Kebutuhan Mikronutrien<\/h3>\n\n\n\n<p>Pasien GGK stadium 3-5 direkomendasikan <strong><span class=\"has-inline-color has-vivid-red-color\">memenuhi angka kecukupan gizi (<em>Recommended Dietary Allowance<\/em>) untuk asupan adekuat semua vitamin dan mineral<\/span><\/strong><\/p>\n\n\n\n<p>Pasien GGK stadium 5 dengan dialisis yang asupan makanannya kurang diberikan suplementasi multivitamin (vitamin larut air &amp; <em>trace elements<\/em>) untuk mencegah defisiensi mikronutrien.  <\/p>\n\n\n\n<p> GGK stadium 3-5 dengan hiperhomosisteinemia terkait penyakit ginjal direkomendasikan suplemen folat rutin untuk menurunkan risiko penyakit kardiovaskular<\/p>\n\n\n\n<p>Vitamin C direkomendasikan 90 mg\/hari pada pria dan 75 mg\/hari pada wanita jika berisiko defisiensi vitamin C<\/p>\n\n\n\n<p>Vitamin A dan E tidak dianjurkan rutin dikonsumsi karena berisiko toksisitas vitamin. Serta tidak dianjurkan pula rutin konsumsi suplemen selenium atau zink. <\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Kebutuhan Elektrolit<\/h3>\n\n\n\n<p id=\"tw-target-text\">Disarankan mengurangi NEAP melalui <strong><span class=\"has-inline-color has-vivid-red-color\">peningkatan asupan sayur dan buah untuk mengurangi laju penurunan fungsi ginjal.<\/span><\/strong><\/p>\n\n\n\n<p id=\"tw-target-text\">GGK stadium 3-4 yang tidak menggunakan analog vitamin D aktif, <strong><span class=\"has-inline-color has-vivid-red-color\">asupan kalsium <\/span><\/strong>(Ca)<strong><span class=\"has-inline-color has-vivid-red-color\"> 800-1000 mg\/hari<\/span><\/strong> (dari makanan, suplemen, dan pengikat fosfat basis Ca) untuk menjaga keseimbangan kalsium.<\/p>\n\n\n\n<p id=\"tw-target-text\"> Asupan fosfor dan kalium juga perlu dijaga supaya kadar serum fosfor dan kalium dalam batas normal.
