Namanya berasal dari bahasa latin <em>carnus<\/em> atau daging, karena senyawa itu diisolasi dari daging pertama kali pada tahun 1905. Karnitin adalah istilah umum untuk sejumlah senyawa yang mencakup L-Carnitine, asetil-L-Carnitine, dan propionil-L-Carnitine. Tetapi hanya L-isomer Carnitine yang aktif secara biologis.<em> L-Carnitine <\/em>bertindak sebagai vitamin dalam cacing gelang (<em>Tenebrio molitor<\/em>) sehingga disebut vitamin BT. Dalam kondisi tertentu, kebutuhan akan L-Carnitine dapat melebihi kapasitas individu untuk mensintesisnya, sehingga menjadikannya sebagai mikronutrien esensial.<span id=\"9fdf5afb-70c5-4f36-a103-62a365f8ec4e\" data-items=\"[&quot;758420385&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b1\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Karnitin adalah molekul endogenous yang mempunyai fungsi dalam metabolisme asam lemak, pada manusia diperoleh baik dari sumber makanan maupun biosintesis endogenous.<span id=\"da6ef007-d03a-4d6c-9f81-ac69800852bf\" data-items=\"[&quot;1217036160&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b2\u200b<\/sup><\/span> <em>Carnitine<\/em> ditemukan dalam jumlah besar di miokardium dan jaringan otot. Substansi ini merupakan faktor esensial dalam transpor asam lemak rantai panjang dari sitoplasma agar dapat melintasi membran dalam mitokondria sampai matriks mitokondria, tempat terjadinya \u03b2-oksidasi.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Fungsi L-Carnitine<\/h2>\n\n\n\n<p>Fungsi utama karnitin adalah memfasilitasi transpor asam lemak rantai panjang (dengan mengaktifkan acylcoenzyme-A esterase) yang ada di sitosol agar dapat melintasi membran dalam mitokondria sampai ke matriks mitokondria, tempat terjadinya \u03b2 oksidasi sehingga mempermudah metabolisme lemak dalam tubuh.<span id=\"238efbe9-b334-49b4-b850-350df23d9ed9\" data-items=\"[&quot;2513738752&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b3\u200b<\/sup><\/span><\/p>\n\n\n\n<p>Karnitin juga mengangkut senyawa beracun yang dihasilkan dari organel mitokondria untuk mencegah akumulasi. Pada manusia sehat biosintesis <em>L-carnitine <\/em>berkisar  0,16 \u2013 0,48 mg\/kg berat badan\/ hari. Seseorang dengan berat badan 70 kg dapat mensintesis antara 11-34 mg karnitin per hari. Tingkat sintesis ini dikombinasikan dengan reabsorpsi <em>L-carnitine<\/em> yang efisien (95%) oleh ginjal, ini sudah cukup untuk mencegah defisiensi L-karnitin pada orang yang secara umum sehat, termasuk vegetarian yang ketat.&nbsp;<span id=\"d443a699-e71f-4122-a9b2-b7ed8780207d\" data-items=\"[&quot;4062915254&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b4\u200b<\/sup><\/span> Asupan asam askorbat yang mencukupi dan pemberian vitamin B<sub>12<\/sub> dapat meningkatkan biosintesis karnitin endogen, sedangkan suplementasi&nbsp; kolin nampaknya menurunkan sintesis karnitin.<span id=\"8608516c-4506-4894-ade3-3ca4dc08891e\" data-items=\"[&quot;665664466&quot;]\" contenteditable=\"false\" data-has-children=\"true\" class=\"abt-citation\"><sup>\u200b5\u200b<\/sup><\/span><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Sumber Makanan<\/li><\/ul>\n\n\n\n<p>Daging, unggas, ikan, dan produk susu merupakan sumber makanan kaya L-karnitin, sementara buah, sayuran, dan biji-bijian mengandung sedikit L-karnitin. Diet omnivora telah ditemukan dapat memberikan sekitar 20-200 mg\/ hari L-karnitin untuk orang dengan berat badan 70 kg, sementara diet vegetarian yang ketat hanya dapat menyediakan sesedikit 1 mg\/ hari untuk orang dengan berat badan 70 kg. Asupan makanan diperkirakan antara 20-300 mg\/hari untuk orang dewasa sehat non-vegetarian dan akan menjadi sekitar&nbsp; 1-3 mg\/hari untuk vegetarian yang ketat. Kebutuhan karnitin dalam jaringan, dan juga konsentrasi karnitinnya, terutama ditentukan oleh tingkat aktivitas oksidasi asam lemaknya.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Defisiensi dan Toksisitas<\/h2>\n\n\n\n<p>Kekurangan karnitine sekunder tidak jarang terjadi dan paling sering dikaitkan dengan dialisis pada gagal ginjal kronis, meskipun juga dapat disebabkan oleh reseksi usus, infeksi berat, dan penyakit hati. Kondisi lain yang terkait dengan defisiensi karnitin termasuk kanker, diabetes, penyakit Alzheimer, dan gagal jantung. Manifestasi patologis dari defisiensi kronis termasuk akumulasi lipid netral dalam otot rangka, otot jantung, dan hati, gangguan serat otot, dan akumulasi agregat mitokondria yang besar dalam otot rangka dan otot polos.
